Please note all sanctuary reservations that utilize sound or multimedia require an authorized CMPHC multimedia tech to be present and require a $75 honorarium for their time. No outside engineers will be permitted.

CULBRETH FACILITY USE AGREEMENT

STATEMENT OF PURPOSE

The church's facilities were provided through God's benevolence and by the sacrificial generosity of church members. The church desires that its facilities be used for the fellowship of the Body of Christ and always to God's glory. Although the facilities are not generally open to the public use, we make our facilities available to approved non-member persons and groups as a witness to our faith, in a spirit of Christian charity, and as a means of demonstrating the Gospel of Jesus Christ in practice.

However, facility use will not be permitted to persons or groups holding, advancing, or advocating beliefs, or advancing, advocating, or engaging in practices that conflict with the church's faith or moral teachings, which are summarized in, among other places, the church's constitution and bylaws. Nor may facilities be used for activities that contradict, or are deemed by the Church Administrative Council as inconsistent with, or contrary to the church's faith or moral teachings. The Administrative Council is the final decision-maker on whether a person or group is allowed to use church facilities.

APPROVED USERS AND PRIORITY OF USE

The Pastor in conjunction with the Administrative Council must approve all uses of church facilities. Priority shall generally be given to members of the church, their immediate families, and organized groups that are part of the ministry, organization, or sponsored activities of the church. Church facilities and equipment will be made available to non-members or outside groups meeting the following qualifications:

1. Groups or persons requesting facility use must affirm that their beliefs and practices and planned uses of the facilities are in harmony with the church's faith and practice.
2. The group or person seeking facility use must submit an online "Culbreth Facility Use Request” and a signed “Culbreth Facility Use Agreement" form.
3. The group or person seeking facility use must be willing to take responsibility for the facilities and equipment used and must agree to abide by the church's rules of conduct for facility use, as stated below and as described in any additional instructions by church staff.

SCHEDULING EVENTS

Requests for facility use must be submitted by filling out the online "Culbreth Facility Use Request.” It can be found on Culbreth Memorial Church’s website, www.culbrethmemorial.com. The event will be reserved and placed on the church calendar only after it has been approved for use and the deductible has been received.

FEES

The cost of facility use is as follows:
•Security & Cleaning Deposit: All use of church facilities is subject to a security and cleaning deposit of $100. This refundable deposit is required to secure every facility reservation and will be returned after a cleaning inspection is performed. A cleaning inspection will be conducted immediately after the scheduled event by a church designee.

•Individual Members in Good Standing: $0 + $100 (Refundable Deposit)
The person hosting the event must be a church member for this to apply. Please note this only applies to church members, not their immediate or extended family who are not members. Church Members are not required to pay for the use of the facilities because the maintenance of the facilities is derived from their faithful tithe and offerings.

•Non-Members:
o Fellowship Hall: $250 + $100 (Refundable Deposit)
o Sanctuary: $250 + $100 (Refundable Deposit)

• Additional Fee’s:

o Stage Setup/Tear down Fee: $75
 An additional Stage Setup/Tear down Fee will be charged of $75 if the Sanctuary stage needs to be cleared prior to an event.

o Media/Sound Tech: $75
 Please note all sanctuary reservations that will utilize sound or multimedia require an authorized CMPHC multimedia tech to be present. This fee will go directly to the technician as an honorarium for their time. No outside engineers are authorized.
o All Fee’s due will need to be turned in by the Sunday before the scheduled event.


CLEAN-UP

1. It is the sole responsibility of the group using the facilities to ensure facilities are cleaned thoroughly and ready for use following the end of their event. All cleanup directions are posted in their respective locations. Church equipment, such as tables and chairs, must be returned to their original placement and arrangement unless otherwise communicated prior to the event.
a. Sanctuary Cleanup Instructions:
i. All worship/media equipment should only be handled by CMPHC Leaders. If clearing the stage is required for an event it is imperative that it be communicated when the event is scheduled. All Worship/media equipment will be removed and reset up by CMPHC leaders only.
ii. All Floors should be vacuumed/swept/mopped in their entirety after use.

INSURANCE

All non-church-sponsored events, person or group using the facilities, may be required to obtain liability Insurance coverage in the amount of at least $500,000.


FACILITY USE AGREEMENT

1. Groups are restricted to only those areas of the facility that the group has reserved.
2. Food and beverages are not allowed in the sanctuary other than water.
3. Church equipment, such as tables and chairs, must be returned to their original placement and arrangement unless otherwise communicated prior to the event.
4. All lights must be turned off and doors locked upon departure.
5. Abusive or foul language, violent behavior, and drug or alcohol abuse are strictly prohibited while using church facilities. Any person exhibiting such behavior will be required to leave the premises. No alcohol or drugs are permitted on church property.
6. Any person or group must sign the “Culbreth Facility Use Request and Agreement" form prior to reservation of church facilities.
7. A thorough clean-up and setup is required by all groups that use the facilities.
 

I AFFIRM THAT:

o I understand that the church does not allow its facilities to be used in a way that contradicts its faith or by persons or groups holding beliefs that contradict the church's faith. To the best of my knowledge, the purpose for which I am requesting the use of church facilities will not contradict the church's faith, and I commit to promptly disclose any potential conflict for which I am aware or become aware to church staff.

o I understand that upon approval of the Facilities Use Request and Agreement, I will need to provide a security and cleaning deposit in the amount of $100. All other fees will be due as noted.

o I understand that I will be responsible for any damages to the church facilities resulting from this proposed use of facilities. User agrees to hold harmless, indemnify and defend Owner (including Owner’s agents, employees, and representatives) from any and all liability for injury or damage including, but not limited to, illness, exposure to infectious/communicable disease, bodily injury, personal injury, emotional injury, or property damage which may result from any person using the above described premises, its entrances and exits, and surrounding areas, for User’s purposes, regardless of whether such injury or damage results from the negligence of the Owner (including Owner’s agents, employees and representatives) or otherwise.

o The church believes disputes are to be worked out between parties without recourse to the courts. See, generally, Matthew Chapter 18 and 1 Corinthians Chapter 6. Accordingly, users of the facility agree to attempt resolution through Christian mediation, and failing that to submit to binding arbitration through a mutually acceptable arbitration service.
